False PhD is a passive item introduced in The Binding of Isaac: Repentance that reverses the standard behavior of pills, turning bad pills into powerful buffs. Found primarily in the Devil Room pool, it provides a unique “high-risk, high-reward” style of gameplay that heavily rewards pill usage. Immediate Effects Upon Pickup Spawns one pill right next to Isaac. Grants +1 Black Heart instantly.

The item fundamentally changes how your character reacts to negative pill effects:

Converts Good Pills to Bad: It turns positive stat-modifying pills into their negative counterparts (e.g., Tears Up becomes Tears Down).

Stat Downs Grant Permanent Damage: Every time you consume a Stat Down pill, you receive a permanent +0.6 Damage Up (+1.2 for Horse Pills). This effect applies retroactively to any bad pills you took before finding the item.

Non-Stat Negative Pills Drop Black Hearts: If you take a negative pill that does not affect raw stats (such as Amnesia, Paralysis, Addicted, or R U A Wizard?), a Black Heart spawns on the floor.

Neutral Pills remain unchanged: Pills like Relax or I Found Pills trigger normally and offer no extra bonuses. Strategic Synergy: Pairing with regular PHD

If you pick up both False PhD and the standard PHD (or Lucky Foot / Virgo), they do not completely cancel each other out. Instead, they create a highly beneficial hybrid system:

Both positive and negative pills will appear in your rotation again.

When you swallow a bad pill, you still get the False PhD permanent damage boost or Black Heart spawn alongside the pill’s normal penalty.
